First Visit

Your initial appointment will consist of a detailed clinical evaluation by Dr. Sridhar, necessary X-rays, diagnosis and a consultation regarding your treatment options. If your treatment plan calls for a root canal, we will schedule another appointment for treatment. If you are in extreme pain, we will do our best to see you, on most occasions, the same day that you call.

Please assist us by providing the following information at the time of your consultation:

• Your referral slip and any X-rays if applicable
• A list of medications you are presently taking
• Completed patient forms – Available Here

If you have dental insurance, bring your current insurance card. If you don’t have one, we will need the subscriber number, group number, and phone number. This will save time and allow us to help you in the processing of your claim.

IMPORTANT:  All patients under the age of 18 years must be accompanied by a parent or guardian to their appointments.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

X-Rays

If your dentist or physician has taken X-rays, please have them forwarded to our office as they may be helpful in our diagnosis. A digital radiograph can be emailed to us. If additional films are necessary, they can be taken at our facility.
